We pray you never need it — but if something comes up while you're living your best life abroad, we need to know exactly who to call.

Your Safety is Our Priority

Listen... your peace of mind on this trip starts before you ever board that plane. We collect emergency contact information for every traveler so that in the rare event of a medical situation, flight disruption, or any unexpected circumstance, MMT can act fast on your behalf — and reach the right person immediately.

Required for all travelers. This form must be completed before your departure date. Your emergency contact will only be reached if you are unreachable and a situation requires it.

What to Have Ready

Before you open the form, have the following handy for your designated emergency contact:

  • Full legal name
  • Relationship to you (spouse, parent, sibling, close friend, etc.)
  • Primary phone number (cell preferred)
  • Secondary phone number (if available)
  • Email address
  • Your booking or reservation name (as it appears on your MMT confirmation)

Your information is kept strictly confidential and used only in the event of a true emergency during your travel dates.
